After you receive notice that you/your child has been accepted to Camp SOAR 2024, please download and complete the necessary pages for Medication & OTC Medication Authorization here:
Additional Items
Additional items needed are Immunization Records, COVID-19 vaccination card, & any Seizure Plans, Behavior Plans, Allergy Plans, or G-Tube Plans. Documents will be uploaded to your child’s CampBrain application.
